Below is an example of what will be needed to fill out the downloadable form to print and fill out. Even if you fill out the form online, you must submit the paper copy (download below) to the Legion.

NOTE:

Once you have completed the downloadable paper form, near the bottom of this page) you must return it to the American Legion, by hand and in person.
